74 is approx 1 year old how come less than 3 rd centile sir.!!
Constipation in early infancy is usually pathological and organic cause usually there so hypothyroidism
Hirshprung and syndromes like downs to be ruled out
Sir should we say right side or areas to be mentioned in exam if signs are present all over chest
On general examination child is acutely sick with tachycardia with tachypnea with poor pulses with hypoxemia
10yr old child with acute febrile illness localizing to respiratory system to rule out pleural or parenchymal pathology with infective cause

Breathlessness can be parenchymal pleural but this was after 8 days of fever so more likely to be complication of immune mediated fever so may be tb pleural effusion
Sir in bacterial can it be low grade bacterial like tuberculosis
Airway is ruled out
Low grade bacterial infection or some treatment given which alter the course
After 5 days if fever persists we should suspect something is wrong
Decreasing fever trend viral
Increasing step ladder like typhoid bacterial infection
On off intermittent fever in malaria
10 yr old child with acute febrile illness localizing to respiratory system most likely pneumonia.!!
