Why not generalized hyperpigmentation in B12 defeciency (why only knuckles and palms involved )

why hyperpigmentation is occuring -Reason ?

Can it be classified as ” Moderate Acute Malnutrition ” with respect to nutritional status?

Why not physiological anemia of infancy and this poor feeding lethargy and abd distension is due to malnurtion

Onest progression and duration of present complaints was very little and negative history and left sided history too long
How was abdominal distension .!!generalized or upper abdomen.
how it was noticed.??

When there is no history of jaundice in immediate neonatal period ,what for phototherapy history aked?!!!
